Case Manager
We are looking for a dedicated Case Manager to join our team in San Francisco, California. In this contract role, you will work closely with tenants to provide essential support and case management services, focusing on housing retention and improving quality of life. This position requires a proactive approach to addressing tenant needs, building community engagement, and collaborating with internal teams and external providers.<br><br>Responsibilities:<br>• Manage a caseload of 60-90 units, providing comprehensive case management services to formerly homeless individuals with diverse challenges, including mental health, substance abuse, and medical issues.<br>• Conduct outreach visits upon tenant entry to housing, during the initial months, and as needed to address ongoing needs.<br>• Perform initial and regular assessments to identify tenant goals and develop strategies to promote housing stability and life improvement.<br>• Support tenants in maintaining housing through consistent engagement, rapport building, and tailored intervention strategies.<br>• Assist tenants with securing and managing benefits, ensuring timely rent payments, and addressing unit habitability concerns.<br>• Organize and lead community-building activities such as social events, tenant groups, and collaborative programs to foster a supportive environment.<br>• Refer tenants to appropriate services, including employment programs, and assist them in successfully accessing and utilizing these resources.<br>• Respond to crises, providing de-escalation support and intervention for tenants experiencing heightened distress.<br>• Collaborate with other departments and external providers to advocate for tenant needs and ensure comprehensive support.<br>• Maintain accurate and confidential records, including tenant files and case documentation, while adhering to reporting protocols.
• Proven experience in case management or a related field, particularly working with vulnerable populations.<br>• Strong administrative skills, including data entry, file maintenance, and organizational abilities.<br>• Ability to conduct needs assessments and develop individualized plans for tenants.<br>• Proficient in crisis management, de-escalation techniques, and conflict resolution.<br>• Excellent communication and interpersonal skills to build rapport with tenants and collaborate effectively with team members.<br>• Knowledge of community resources and programs to support tenant goals.<br>• Familiarity with housing retention strategies and tenant advocacy.<br>• Commitment to maintaining confidentiality and professionalism in all interactions.
