Case Manager
We are looking for a dedicated Case Manager to join our team in San Francisco, California. In this contract role, you will provide comprehensive support to individuals facing housing challenges, mental health concerns, and other barriers. Your focus will be on fostering stability, promoting community engagement, and ensuring tenants have access to the resources they need. This is an opportunity to make a meaningful impact while working collaboratively within a dynamic team.<br><br>Responsibilities:<br>• Manage a caseload of 60-90 units, supporting formerly homeless individuals with diverse needs, including mental health challenges and substance abuse issues.<br>• Conduct outreach visits upon tenant entry, regularly following up to assess needs and provide ongoing case management services.<br>• Assist tenants in retaining housing by building rapport, offering frequent engagement, and implementing effective intervention strategies.<br>• Guide tenants in accessing benefits, managing rent payments, and addressing habitability concerns through inspections and pest control checks.<br>• Organize community-building activities such as tenant groups, social events, and collaborative programs to foster a supportive environment.<br>• Connect tenants with employment opportunities and relevant services, ensuring successful follow-through and engagement.<br>• Respond to tenant crises with de-escalation techniques and provide necessary intervention and support.<br>• Collaborate with internal departments and external providers to advocate for tenant needs and ensure comprehensive care.<br>• Maintain accurate and confidential tenant records, including case files and documentation of services provided.<br>• Report and address incidents of abuse or neglect in accordance with legal requirements and organizational protocols.
• Proven experience in case management or a similar role, ideally working with vulnerable populations.<br>• Strong administrative skills, including data entry, filing, and record maintenance.<br>• Ability to conduct thorough needs assessments and develop personalized service plans.<br>• Excellent interpersonal and communication skills to build trust and rapport with tenants.<br>• Knowledge of housing retention strategies and community resources.<br>• Crisis management skills, including de-escalation techniques.<br>• Proficiency in organizing and hosting community events and tenant engagement activities.<br>• Understanding of confidentiality protocols and ethical standards in social services.
