Case Manager
We are looking for a dedicated Case Manager to join our team in San Pedro, California, supporting a non-profit organization. In this Contract to permanent employment role, you will play a key part in assisting individuals and families by providing tailored behavioral health services. This position offers a meaningful opportunity to make a difference in the lives of those in need while working collaboratively with clients and their support networks.<br><br>Responsibilities:<br>• Conduct initial screenings and comprehensive mental health assessments to identify service needs and recommend appropriate care options.<br>• Share assessment results with clients and their families, ensuring clear communication and understanding.<br>• Collaborate with clients and their families to develop personalized treatment plans that include actionable goals, objectives, and interventions.<br>• Provide individual, family, and group therapy sessions to address identified behavioral health needs.<br>• Maintain detailed and accurate records of client progress and treatment plans.<br>• Offer ongoing case management services to ensure clients receive consistent and effective support.<br>• Act as a liaison between clients and community resources to enhance access to services.<br>• Monitor and evaluate the effectiveness of treatment plans, adjusting strategies as needed to achieve optimal outcomes.<br>• Communicate effectively with multidisciplinary teams to coordinate care and ensure alignment of services.
<p>• Minimum of 1 year of experience in case management or a related field.</p><p>• Strong customer service skills with the ability to build rapport and trust with clients.</p><p>• Knowledge of behavioral health principles and therapeutic strategies.</p><p>• Proven ability to conduct assessments and develop effective treatment plans.</p><p>• Experience providing individual, family, and group therapy services.</p><p>• Excellent communication and interpersonal skills to engage with clients and collaborate with teams.</p><p>• Ability to manage multiple cases effectively while meeting deadlines.</p><p>• Familiarity with community resources and support services.</p><ul><li>Bilingual Spanish a plus! </li></ul>
