Intake Coordinator
We are looking for an organized and detail-oriented Intake Coordinator to join our team in White Plains, New York. This role is a Contract-to-permanent position, offering a unique opportunity to contribute to patient care and referral management. The ideal candidate will excel at handling referrals, collaborating with healthcare professionals, and ensuring a seamless intake process.<br><br>Responsibilities:<br>• Process patient referrals using electronic medical records, ensuring accurate entry of demographics, clinical details, and reimbursement information.<br>• Review clinical documents to verify patients meet criteria for services offered.<br>• Schedule service dates with patients, update intake boards, and coordinate with relevant teams.<br>• Verify insurance eligibility and benefits across Medicare, Medicaid, and commercial policies through online platforms.<br>• Screen incoming referrals, determining appropriateness for home care services.<br>• Collaborate with intake staff to collect necessary information for each referral.<br>• Participate in weekend rotation schedules to support intake operations during weekends and holidays.<br>• Build and maintain relationships with medical practices to foster patient referrals.<br>• Perform additional duties as assigned to support the department's goals.
• High school diploma or equivalent; an associate degree is preferred.<br>• Proficiency in data entry and familiarity with electronic medical record systems.<br>• Prior experience with health insurance processes and verification.<br>• Strong understanding of medical terminology and treatment plans.<br>• Exceptional customer service skills and ability to follow up effectively.<br>• Excellent organizational abilities; case management experience is an advantage.<br>• Knowledge of medical terminology and coding systems such as ICD-10.<br>• Ability to adapt to physical demands, including sitting, standing, or lifting up to 15 pounds occasionally.
