Case Manager
We are looking for a compassionate and organized Case Manager to support formerly homeless adults in San Francisco, California. This Long-term Contract position focuses on helping residents maintain stable housing, connect with essential services, and improve overall well-being through consistent, person-centered case management. The ideal candidate brings strong administrative support skills, sound judgment in challenging situations, and the ability to build trust with tenants, property staff, and community partners.<br><br>Responsibilities:<br>• Oversee an assigned portfolio of residents, providing consistent case management support tailored to individual housing stability and wellness goals.<br>• Meet with tenants regularly through outreach, intake conversations, and ongoing check-ins to assess needs, monitor progress, and adjust support plans as appropriate.<br>• Help residents secure and maintain public benefits, stay current with rent obligations, and address barriers that may affect continued housing.<br>• Coordinate referrals to community-based resources such as health care, behavioral health, substance use services, and employment programs, while encouraging successful follow-through.<br>• Partner with property management during unit walkthroughs, pest control visits, and habitability-related concerns to support safe and stable living conditions.<br>• Foster a positive tenant community by organizing group activities, resident meetings, and engagement events in collaboration with staff and tenants.<br>• Intervene during tenant crises by using de-escalation techniques, documenting incidents, and connecting individuals with appropriate support services.<br>• Maintain accurate case records, complete required documentation, and safeguard confidential client information in accordance with agency standards.<br>• Work closely with internal teams and external service providers to advocate for residents and report any suspected abuse or neglect through proper channels.<br>• Assist with site-based operational support activities, including resident recertification efforts, check disbursement days, and attendance at scheduled meetings.
• Experience providing case management, administrative support, or direct services to vulnerable populations in housing, social services, or community-based settings.<br>• Ability to manage a high-volume caseload while maintaining accurate documentation, organized files, and timely follow-up.<br>• Strong interpersonal skills with the ability to build trust, communicate clearly, and work effectively with residents from diverse backgrounds.<br>• Confidence handling crisis situations with sound judgment and calm de-escalation techniques.<br>• Proficiency in administrative tasks such as data entry, recordkeeping, and filing.<br>• Knowledge of community resources, public benefits, and service referral practices that support housing retention and client stability.<br>• Ability to collaborate with multidisciplinary teams, property staff, and outside partners to coordinate care and tenant support.<br>• Commitment to confidentiality, attention to detail, ethical standards, and respectful client-centered service delivery.
