<p>We are seeking a Case Manager to coordinate services, support clients, and help ensure timely and effective case resolution.</p><p><strong>Responsibilities</strong></p><ul><li>Assess client needs and develop service plans</li><li>Coordinate referrals, resources, and follow-up care or services</li><li>Maintain accurate case files and documentation</li><li>Monitor client progress and update plans as needed</li><li>Communicate with clients, families, and service providers</li><li>Ensure compliance with organizational and regulatory requirements</li></ul>

<p><strong>Requirements</strong></p><ul><li>Bachelor’s degree in social work, human services, or related field preferred</li><li>Prior case management or client service experience preferred</li><li>Strong interpersonal, documentation, and problem-solving skills</li><li>Ability to manage sensitive and confidential information</li><li>Knowledge of community resources and service coordination is a plus</li></ul><p><br></p>
