<p>Position Overview</p><p>The ICMS Housing Navigator provides intensive case management and housing navigation services to families experiencing homelessness. This role focuses on housing stabilization, resource coordination, and long-term self-sufficiency through individualized, trauma-informed support.</p><p>Key Responsibilities</p><ul><li>Provide intensive case management services to families enrolled in housing programs</li><li>Conduct comprehensive assessments to identify housing needs, barriers, and strengths</li><li>Develop and monitor individualized housing stability and service plans</li><li>Assist clients with housing searches, applications, and documentation</li><li>Coordinate referrals to community resources including healthcare, benefits, education, and supportive services</li><li>Advocate on behalf of clients with landlords, housing providers, and partner agencies</li><li>Maintain accurate and timely case notes, service plans, and outcome documentation</li><li>Conduct home visits and community-based meetings as required</li><li>Support clients in maintaining housing stability and achieving long-term independence</li></ul><p><br></p>
