<p>Essential Duties &amp; Responsibilities</p><ul><li>Conduct comprehensive participant <strong>intakes, assessments, and housing needs evaluations</strong> and develop individualized Housing Case Plans based on each family&#39;s circumstances, barriers, and goals.</li><li>Provide ongoing, client-centered case management services designed to promote <strong>housing stability, wellness, independence, and self-sufficiency</strong>.</li><li>Connect participants with appropriate community resources and supportive services, including <strong>mental health services, substance use treatment, domestic violence resources, childcare, employment assistance, benefits, and other social services</strong>.</li><li>Make referrals to community-based programs and follow up with participants and service providers to ensure successful connection and completion of services.</li><li>Track participant progress toward established housing and self-sufficiency goals and modify case plans as appropriate.</li><li>Complete participant <strong>recertifications every three months</strong> to evaluate continued eligibility, progress, housing stability, and ongoing need for assistance.</li><li>Educate participants regarding the program&#39;s <strong>rental assistance step-down model</strong>, including the gradual reduction of rental subsidies and expectations for increasing financial independence.</li><li>Assist participants throughout all stages of the permanent housing process, including identifying available housing, completing applications, gathering required documentation, communicating with landlords, advocating on behalf of participants, and coordinating leasing and move-in activities.</li><li>Work collaboratively with <strong>Housing Locators/Matchers</strong> to identify appropriate housing opportunities and overcome barriers to placement.</li><li>Develop and maintain relationships with <strong>property owners, landlords, and property managers</strong> to expand available housing opportunities for program participants.</li><li>Educate landlords and property managers regarding UBH rental assistance and placement programs, including <strong>short-, medium-, and long-term subsidy structures, program timelines, payment processes, and landlord participation requirements</strong>.</li><li>Communicate and negotiate with landlords regarding rental terms, participant eligibility, leasing requirements, and other housing-related matters.</li><li>Perform <strong>rent calculations</strong> for rental assistance programs and counsel participants regarding rent obligations, security deposits, lease provisions, and other housing-related financial responsibilities.</li><li><br></li></ul>

<p>Key Qualifications &amp; Skills</p><ul><li>Experience providing <strong>case management, housing navigation, homeless services, social services, or supportive services</strong> to individuals and families.</li><li>Knowledge of <strong>rapid re-housing, rental assistance, housing stabilization, and homelessness prevention programs</strong> preferred.</li><li>Experience working with families involved with <strong>child welfare, Family Reunification, or Family Maintenance services</strong> is highly desirable.</li><li>Understanding of <strong>tenant rights and responsibilities, landlord relations, lease requirements, Fair Market Rent guidelines, and eviction-prevention practices</strong>.</li><li>Ability to establish professional relationships with landlords, property managers, community organizations, and social service providers.</li><li>Strong assessment, crisis intervention, problem-solving, advocacy, and conflict-resolution skills.</li><li>Ability to work effectively with families experiencing homelessness and individuals facing multiple barriers to housing stability.</li><li>Strong organizational and documentation skills with the ability to manage a caseload and meet established deadlines.</li><li>Experience with <strong>HMIS, CareLinQ, or comparable case management databases</strong> preferred.</li><li>Strong written and verbal communication skills.</li><li>Ability to maintain participant confidentiality and exercise sound professional judgment when handling sensitive information.</li></ul><p><br></p>
